Before Your Appointment

General Information

 

If this is the first time that you are visiting our clinic, please plan on arriving at the office 10 minutes before your appointment. Our office can sometimes be difficult to find if you are not familiar with the area.

 

If possible, please leave siblings at home, or plan on bringing another set of hands to keep them occupied during your appointment. Appointments can sometimes be fairly lengthy, and it can be difficult to engage in a full discussion about your child's issues without both of our undivided attention. 

 

If your child is being seen for a developmental, school-related, or behavioural concern (including anxiety, depression, autism, ADHD, etc.), please provide the following documents for us, either by bringing them to your appointment or faxing them to the office beforehand.

         

              letters/notes from the teacher outlining his/her concerns

              all previous report cards (going back as far as kindergarten, if possible)

              any school assessments that have been done

              any speech and language, occupational therapy, and physiotherapy assessments that have been done

 

If you are unable to find all of your child's previous report cards or school assessments, please ask the school to provide you with copies, or have them fax the documents directly to the office.

 

By providing us with the above information, this helps us immensely in getting to know your child, and helps us provide a more complete assessment.

Reminder Calls

 

Our MOAs try provide reminder phone calls 1-2 days before your appointment. However, it is also your responsibility to remember when your child's next appointment is for. 

Rescheduling Your Appointment

 

Please notify the office at least 48 hours before your upcoming appointment if you are unable to attend. This allows us to schedule another patient into your timeslot. This also helps us keep our wait-lists down to a manageable timeframe.

 

No-shows or Late Cancellations

No-shows or late cancellations will be charged a fee depending on the length of time scheduled for your appointment. No-shows/late cancellations for appointments booked for 1 or more hours will be charged $50, while no-shows/late cancellations for 30 minute appointments will be charged $25. These fees will need to be paid before the appointment is rebooked.
